Enter your activation code and other information such as your Social Security Number and date of birth to instantly activate your account and create a new user ID and password.
2-Request a Bon Secours My Chart Activation Code:
If you don’t have an activation code then you can use the https://mychart.mybonsecours.com/mychart/Signup link to create a new account. Some information will be required in order to grant you a MyChart account. You will have to enter your demographic information in the online signup form.
