Ascension has a national presence and provides its healthcare services in different states. You must have the correct web URL of your patient portal. To help you out, we are sharing details on how to find your patient portal.

  • Please visit the Ascension website https://healthcare.ascension.org/.
  • Click the Patient Portal link in the top bar.
  • Alternatively, Visit the patient portal web page directly at https://healthcare.ascension.org/patient-portal if the link in the top bar is not working,
  • On the patient portal web page, select your state (Wisconsin).

When a patient visits any Ascension healthcare facility for the first time then Ascension shares an activation code with the patient.
